WHEN TO USE THIS FORM

Use this Engagement for documentation imaging tied to insurance claims — residential or commercial loss, post-disaster scans, scope-of-damage 3D capture, and pre/post-restoration photography. The Client is typically a public adjuster, restoration contractor, attorney, or property owner pursuing a claim. This form addresses the unique needs of claims work: chain-of-custody, metadata preservation, evidence-grade authenticity, and payment terms keyed to claim settlement.

This Photography for Insurance Claims Engagement (this “Engagement”) is entered into and effective as of [EFFECTIVE DATE] by and between [PHOTOGRAPHER LEGAL NAME] (the “Photographer”), and [CLIENT NAME], a [PUBLIC ADJUSTER / RESTORATION CONTRACTOR / ATTORNEY / PROPERTY OWNER / INSURED] (the “Client”).

1. Scope of Documentation Services

Photographer will produce documentation imaging of the loss at [PROPERTY ADDRESS] (the “Property”) in support of insurance claim [CLAIM NUMBER OR — TBD —] against carrier [CARRIER NAME]. Services may include:

  • HDR interior and exterior photography of damaged areas.
  • 3D digital twin / scan of the loss for measurement and dimensional evidence (Matterport, Leica BLK, iPhone+RoomPlan, or equivalent).
  • Aerial / drone photography of roof, structural, or large-scale exterior damage (FAA Part 107).
  • Pre- and post-restoration comparison imagery.
  • Date- and GPS-stamped contact sheet with embedded metadata.
  • Affidavit of authenticity from Photographer (Schedule A.4).
