It documents a specific scope change against an existing service agreement, capturing the change description, cost delta, revised total fee, and updated delivery timeline. Both parties sign before any modified work begins, creating a clear written record that supplements — but does not replace — the original contract.

When should you use this form?

Use the Change Order Form whenever a client requests additions or modifications after the original service agreement is signed — for example, adding a Matterport 3D tour to a photography-only booking, expanding square footage coverage mid-shoot, swapping a standard photo package for aerial drone add-ons, or rescheduling a shoot that alters agreed deliverable dates. It is especially useful when the cost, timeline, or deliverables of the original engagement are affected and written mutual acknowledgment is needed before work resumes.
